  • Caregiver License: Oklahoma residents who are 18 and older and who regularly look over a medical marijuana patient. Valid until the associated licensed patient(s) expiration date, not exceeding two years.
  • Out-of-State Patient License: Residents of other states with state-government-issued medical marijuana licenses. Valid up to 30 days.
  • Short-Term Licenses: Oklahoma residents need medical marijuana for a shorter duration. Valid up to 60 days.
Application Requirements
  • Adult Patient License: Proof of identity, proof of residency, acceptable photo, and a signed Physician Recommendation Form (Adult Patient) from an authorized physician.
  • Minor Patient License: Proof of identity, proof of residency, acceptable photo, and a Physician Recommendation Form (Minor Patient) signed by two qualified physicians.
  • Caregiver License: Proof of identity, proof of residency, and an acceptable photo.
  • Out-of-State Patient License: State-issued medical marijuana patient license, an acceptable photo, and proof of identity (digital color copies of the front and back of a state-issued driver’s license or ID card or a U.S. Passport).
  • Short-Term Licenses: Proof of identity, proof of residency, acceptable photo, and a signed Physician Recommendation Form (Adult Patient) from an authorized physician.
Application Fees
  • Adult Patient License: The nonrefundable application fee is $100 plus a $4.30 credit card processing fee or $20 plus a $2.50 credit card processing fee for individuals with proof of enrollment in Medicaid (SoonerCare) or Medicare or status as a 100% disabled veteran.
  • Minor Patient License: The nonrefundable application fee is $100 plus a $4.30 credit card processing fee or $20 plus a $2.50 credit card processing fee for individuals with proof of enrollment in Medicaid (SoonerCare) or Medicare.
  • Caregiver License: No application fee.
  • Out-of-State Patient License: The nonrefundable processing fee is $100 plus a $4.30 credit card processing fee.
  • Short-Term Licenses: The nonrefundable application fee is $100 plus a $4.30 credit card processing fee or $20 plus a $2.50 credit card processing fee for individuals with proof of enrollment in Medicaid (SoonerCare) or Medicare or status as a 100% disabled veteran.
